By pulling the jaw forwards, the upper airway space might be widen both anterior-posteriorly and medio-laterally. Therefore, sleep disordered breathing (SDB) patients possess anatomical or physiological factors facilitate the above-mentioned configurational changes, should have good prognosis treated by mandible advancing devices (MAD). It seems that the tissue tone of the pharyngeal walls is the key factor governing the medio-lateral widening of the pharyngeal walls. The posterior dropping of the tongue would compromise the anterior-posterior increasing of the airway space. Clinically BMI, posture-related AHI from the PSG, and CPAP titration pressure can be used to estimate the extent of the airway space increasing through jaw protrusion. Recently, upper airway sonogram study can visualize the tendency of tongue dropping while the jaw is protruding. All these factors should be taken into account while deciding the appropriate treatment options of SDB patients. |
