Abstract:
Contemporary maxillomandibular advancement (MMA) surgery for obstructive sleep apnea (OSA) has been the first option when skeletal surgery is considered. Despite a high success rate and high reduction rate of the apnea-hypopnea index, there is still a lack of standardized pre-surgical evaluation and treatment planning procedures. In addition to the gold standard of polysomnography, commonly used tools include clinical craniofacial evaluation, 2D/3D photo analysis, 2D/3D cephalometry and airway dimension measurements, computational fluid dynamics study, and drug-induced sleep (sedative) endoscopy. Through multi-tier pre-surgical evaluations, we may obtain a comprehensive assessment to connect anatomy to sleep physiology and build up a rational treatment plan for individual patients.
