Obstructive Sleep Apnea (OSA) affects approximately 15% of adults in Taiwan, presenting a significant public health challenge. The gold standard for diagnosing OSA has traditionally been the in-lab Polysomnography (PSG); however, the high prevalence of OSA and the associated costs and logistical challenges necessitate alternative diagnostic methods. Home Sleep Apnea Testing (HSAT) offers a cost-effective and efficient screening tool, particularly for adults at high risk for moderate to severe OSA without comorbidities.
The 2024 HSAT guidelines by the Taiwan Society of Sleep Medicine aim to provide sleep professionals with standardized procedures for utilizing HSAT in clinical practice. These guidelines emphasize the importance of proper patient selection, indicating HSAT primarily for those with a high risk of moderate to severe OSA and no significant comorbid conditions, such as central sleep apnea or severe cardiovascular diseases. The guidelines also delineate contraindications, including severe heart or lung diseases, opioid use, and uncontrolled psychiatric disorders.
HSAT equipment should be managed by qualified sleep technicians under the supervision of certified sleep physicians. The equipment must meet specific technical requirements, including the ability to record physiological parameters ensuring the data's diagnostic validity. Additionally, the guidelines recommend the involvement of sleep technicians in patient education, device setup, and data interpretation to reduce the incidence of test failures and enhance the accuracy of the results.
The 2024 guidelines provide a comprehensive framework for the effective and safe implementation of HSAT, ensuring high diagnostic accuracy while maintaining patient safety and data integrity. By adhering to these guidelines, sleep centers in Taiwan can enhance the management of OSA, offering timely and accessible care to patients in need.
